By entering into this Agreement, I represent that I regularly see my own dental care professional and have no untreated dental conditions; I will continue to see a dentist while undergoing this cosmetic enhancement. I understand that unlike traditional braces, ProClear™ Aligners are custom trays that may improve and brighten smiles at a lower cost than traditional braces. I instruct ProClear™ to fabricate a series of clear aligners that will cosmetically enhance my smile. By purchasing the ProClear™ Aligner Kit, I am agreeing to these terms and conditions.
